JOB DESCRIPTION SUMMARY
The Licensed Clinical Social workers help people solve and cope with problems in their everyday lives. A licensed Clinical social worker diagnoses and help treat mental, behavioral, and emotional issues.
ESSENTIAL JOB FUNCTIONS/ RESPONSIBILITIES
- Collect information about clients through interviews, test and evaluation of day to day functioning within work, family and other routines identifying areas needing continued support, resources and treatment in order to assist clients.
- Work in collaboration with other healthcare givers to evaluate the medical or physical condition of the patients
- Works with the patient’s family and other service providers to develop a plan for care for the patient.
- Counsel patients on how to overcome their conditions and avoid dependencies as they recuperate from their problems.
- Provide therapeutic services to clients through individual, family and group sessions that are consistent with the current research, professional standards and values
- Prepare and complete diagnostic and biopsychosocial assessments, clinical notes, referrals, treatment plans of service recipients.
- Regularly review and recommend changes in program services to meet service recipient needs with particular emphasis on services for underserved populations, oversight of program evaluation and quality assurance functions for departments including assessment of applicable legal and professional requirements and monitoring treatment plans for service recipients.
- Adheres to agency values and principles and promotes them within the agency and the community and works to position the agency as a leader in service provision in the community.
- Ensures that accurate program and client records are maintained and that confidentiality is a priority. Ensures that all pertinent documentation is completed and safely stored.
- Maintains confidentiality with regard to service recipients’ treatment and records in accordance with HIPAA.
- Maintains a current awareness of developments in assigned field and within the agency.

Job Title: Clinical Director
Reports To: Executive Director/Administrator
JOB DESCRIPTION SUMMARY
The Clinical Director is responsible for the overall direction of ABUNDANT GRACE HEALTH SERVICES, LLC Clinical Services.
The Clinical Director establishes, implements and evaluates goals and objectives for Case
Management thatmeet and promote the standards of quality and contribute to the total organization and philosophy.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
- Coordinates and oversees all direct and indirect consumers services provided by clinical organization personnel.
- Provides guidance and counseling to coordinators and Clinical Supervisors to assist them in continually improving all aspects of mental health care services, provided through organization personnel.
- Assists Clinical Supervisors in managing clinical teams and planning.
- Provides help in assessment, planning, implementation and evaluation of consumer and family.
- Assists the Executive Director/ Administrator in the preparation and administration of the organization’s budget.
- interprets operational indicators to detect census changes and increases or decreases in volume, which could impact staffing levels, revenues or expenses.
- Evaluates performance of Clinical Supervisors.
- Assists Clinical Supervisors to develop skills and techniques in evaluating the performance of clinicians.
- Hires, evaluates, and terminates organization personnel.
- Conducts clinical performance evaluations annually, or more frequently if indicated.
- Oversees the maintenance of patient clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities.

POSITTON QUALIFICATIONS
- Licensed independent clinical social worker (Licsw)
- Bachelor’s degree in Nursing from an accredited program by the National League for Nursing. Master’s degree in social workerpreferred.
- Three years of management experience in a health mental or related health care organization.
- Demonstrated ability to supervise and direct professional administrative personnel.
- Ability to market and deal tactfully with consumers and the community.
- Has excellent observation, verbal and written communication skills.
Job Title/ Position: Referral /Intake coordinator
Reports To: Executive Director/ Administrator
JOB DESCRIPTION SUMMARY
Responsible for managing all aspects of the consumer intake process including managing the members of the intake team, establishing and maintaining positive relationships with consumers and referral sources, responding to consumer requests and concerns, and managing the insurance verification and authorization processes.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
- Directs all daily consumer referral and intake operations including providing direct oversight of the establishment and implementation of intake policies.
- Ensures compliance with all state, federal, and EPD referral/intake regulatory requirements.
- Directs the implementation of improved work methods and procedures to ensure consumersare admitted in accordance with policy.
- Establishes and maintains positive working relationships with current and potentialreferral sources.
- Ensures maximum third-party reimbursement through direct oversight of insuranceverification and authorization processes.
- Negotiates service pricing with insurance Case Managers and other payers withinestablished financial and credit parameters.
- Assists the Executive Director/Administrator in the preparation of an annual budget for theintake department and monitors allocation of resources according to budgetary limitations.

POSITION QUALIFICATIONS

The Incumbent will assist clients in their physical, social, emotional and daily life skills development. With the aim of increasing their independence and allowing them to function appropriately in the community. This position responds to the clinical manager.
ESSENTIAL JOB FUNCTIONS/ RESPONSIBILITIES
- 1- Performs medically necessary community support, care coordination; therapeutic and skill building activities identified in the client treatment/care plan that’s developed and implemented.
- Guides and supports clients in the development of skills and strategies for addressing personal situations and to help them manage them. Supports clients in achieving recovery and building resilience.
- Documents and maintains confidential treatment records and reports in a timely manner, within 24 hours of service delivery.
- Collaborates with staff and external resources that will support the client to perform clinical assessments and develop treatment plans.
- Supports clients individually and in group, sessions to assist in overcoming dependencies, adjusting to life, and making changes through skill development.
- Acts as client advocate in order to coordinate required services or to resolve emergency problems in crisis situations.
- Meets with families, probation officers, police and other interested parties in order to exchange necessary information during the treatment process.
- Supports family members to assist them in understanding and supporting clients or patients.
- Refers patients, clients or family members to community resources or to specialists as necessary.

JOB DESCRIPTION SUMMARY
The secretary/receptionist is responsible for routine clerical matters and assuring that
documents flow in a coordinated and efficient manner.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
- Provides assistance to the Executive Director/Administrator, preserving the confidentialnature of items of which he/she has knowledge.
- Maintains the files, supplies, postage and general office condition in an orderly manner.
- Handles all correspondence and word processing for the home health organization.
- Maintains minutes of meetings.
- Answers telephone inquiries and channels them appropriately.
- Files clinical notes after recording on the “weekly ledger,’sheet.
- As necessary, sends physician’s orders for signature. Keeps current log of what is sentand tracks receipt of signed document.
- Closes patient’s clinical record when discharged.
- Maintains current patient statistics utilizing the Management lnformation System.
- Maintains ledger cards for all new patients admitted for clerk,s use.
